Do It Online

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results - currently we can provide your medical records from 01-10-2019 onwards.

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

We actively encourage our patients to make use of our online services wherever possible.


How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up.

Are you new to online access or not even sure where to start? Just contact the surgery and ask to speak with our reception team who will be happy to help you. They can arrange for you to come in to the surgery to show you the basics - We are also open on Wednesday evenings or Saturday mornings our Broadway surgery if you'd like to come on down. 


Registering for online services

To register for Patient Access for yourself, please use the Online access registration form and return it to reception or via email (ingol.broadway@nhs.net) with either photo ID or proof of address.

If you do not have photographic I.D. you may be asked to verify your identity by answering questions pertaining to your medical record.


Requesting Access on behalf of someone else (Proxy-Access)

If you would like to access records on behalf of someone else, either as a parent/guardian or as a carer, then we would ask that you complete the Patient Online - Proxy consent form. We will need to ensure that the patient has given consent unless they are a child or do not have capacity to consent.

Before giving access to someone else, we would recommend reading this leaflet.

Once your application is completed and approved, you will be issued with a registration document. You will need to follow this document to complete your registration online.

Photo ID and proof of address will be requested by our team so we can verify both the proxy user and the patient.
