Do It Online

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up.

Are you new to online access or not even sure where to start? Just contact the surgery and ask to speak with our reception team who will be happy to help you. They can arrange for you to come in to the surgery to show you the basics - We are also open on Wednesday evenings at our Broadway surgery if you'd like to come on down. 

Download the new NHS App 

All you need to do is head to your smartphone or tablet App Store, search for the NHS App and follow the instructions on how to create an account. You can either pop in to the surgery with some identification and our reception team can print your off your registration credentials, or the app will allow you to send your own identification online and will set up an account for you in just a few minutes.

You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.

If you already use other online access services, you can continue to use it. You can use the NHS App as well.nhs app

You could also download the Patient Access or the MyGP App to access GP appointments, prescriptions and to also view part of your medical record.

Registering for online services

To register for Patient Access for yourself, please use the Patient Online Records Application Form and return it to reception. Please be aware that you will be asked to verify your identity. You can do this by providing 2 forms of identification, one of which should be photographic I.D.

If you do not have photographic I.D. you may be asked to verify your identity by answering questions pertaining to your medical record.

Requesting Access on behalf of someone else (Proxy-Access)

If you would like to access records on behalf of someone else, either as a parent/guardian or as a carer, then we would ask that you complete the Patient Online - Proxy consent form. We will need to ensure that the patient has given consent unless they are a child or do not have capacity to consent.

Before giving access to someone else, we would recommend reading this leaflet.

Once your application is completed and approved, you will be issued with a registration document. You will need to follow this document to complete your registration online.
